Create document that explains the evaluation protocol

This document follows closely with it's corresponding chapter in the project proposal. Currently the template that is used in this repo is lacking clarification of the questions. And the project proposal is a less dynamic document.

In the end this document will contain all the questions with clarification. But also contain a changelog that explains the when the question was added. With the goal to explain why and at what point the question was added, and why not earlier.
